Apply For Membership Card

Please choose from the following two types of membership cards

• Platinum Membership Card - used in conjunction with any form of payment. This is a private label or co-branded card sponored by the membership organization you are joining.
• Diamond Membership Card - this is a debit card. The application requires a drivers license or social security number. The card comes with a VISA or Mastercard Logo, a hologram and is good at any ATM or point-of-sale worldwide, including Internet transactions.
